The name does not measure the diameter of the skin lesion or automatically mean early disease. Literature describes overlap with <a href=\"https:\/\/www.ilmelanoma.com\/en\/melanoma\/rare-variants-of-melanoma\/nevoid-melanoma\/\">nevoid melanoma<\/a> because the cells can resemble those of a benign nevus.<\/p>\n<p>It is a rare presentation defined histologically. It should not be confused with small cell carcinoma or Merkel cell carcinoma: cellular appearance may require differential diagnosis, but the tumours have different origins.<\/p>\n<\/section>\n<section class=\"im26-dg-section\" id=\"rf-diagnosi\">\n<h2>Why it needs careful microscopic assessment<\/h2>\n<p>Descriptions include compact nests of cells with scant cytoplasm and dark nuclei, sometimes associated with an epidermal melanoma-in-situ component. Resemblance of the dermal component to a nevus can be misleading, especially in partial samples.<\/p>\n<p>Architecture, atypia, maturation and relationships between components must be assessed together. Melanocytic markers may support diagnosis, but their distribution can vary within the tumour. Comparison with other small-cell neoplasms requires a panel selected by the pathologist.<\/p>\n<p>Published series are limited. Age, sex or site patterns observed in a few patients do not become exclusion criteria. Expert review may help when the report remains uncertain.<\/p>\n<\/section>\n<section class=\"im26-dg-section\" id=\"rf-percorso\">\n<h2>From diagnosis to care<\/h2>\n<p>A suspicious lesion is assessed clinically and sampled when indicated; self-examination cannot determine cellular size. After diagnosis, site, Breslow thickness, ulceration, margins and spread guide decisions.<\/p>\n<p>Management follows melanoma principles appropriate to the case, with <a href=\"https:\/\/www.ilmelanoma.com\/en\/guides\/treatment\/\">surgery<\/a> for localised disease and additional investigations or treatment when needed. There is no separate standard therapy selected solely because the cells are small.<\/p>\n<p>Prognosis cannot be inferred from the name. Ask whether diagnosis is definitive, what the stage is and whether further sampling or slide review is needed.
