Dabrafenib inhibits mutated BRAF; trametinib inhibits MEK, a later step in the MAPK pathway. In some melanomas this signalling chain promotes tumour-cell proliferation. Blocking it at two points provides the rationale for the combination.

They are not immunotherapies and are not conventional cytotoxic chemotherapy. Their use requires a relevant mutation documented in the tumour: a melanoma diagnosis alone is insufficient. Target selectivity does not mean no effects on healthy tissues or a guaranteed response. Targeted treatment also requires monitoring and can cause important adverse effects.

The medicines have complementary roles but different instructions and safety profiles. The combination must be the prescribed one: independently substituting another BRAF or MEK inhibitor for a component is inappropriate.

Sources for this section: EMA — Tafinlar: indications, product information and leaflet; EMA — Mekinist: indications, product information and leaflet.

European indications and access in Italy

The combination is an established option for unresectable or metastatic melanoma with a BRAF V600 mutation. It also has an adjuvant indication in completely resected BRAF V600 stage III melanoma. In the updated EMA documentation consulted, melanoma indications include adults and adolescents aged 12 or older; adolescent assessment and dosing require specific expertise and cannot be inferred from adult regimens.

Authorisation does not mean everyone with the mutation should receive this pair, or that every use is automatically reimbursed under identical conditions in Italy. The centre checks the indication, AIFA criteria, any monitoring registries and alternatives. Age, stage and variant must be interpreted within current product information.

Using the same medicines before surgery or in a different combination is a separate question: an indication cannot be extended simply by analogy.

Sources for this section: EMA — Tafinlar: indications, product information and leaflet; EMA — Mekinist: indications, product information and leaflet; AIFA — Reimbursement and pricing.

BRAF V600E, V600K and interpreting the report

BRAF V600 must be confirmed with a validated test before treatment. Material may come from the melanoma or a metastasis, depending on availability and laboratory assessment. The report must specify the variant: BRAF V600E and V600K are not interchangeable with any gene alteration, and non-V600 mutations require different considerations.

Specimen quality and what the method can detect matter. If a result cannot be interpreted, the team may request further analyses; it must not automatically be equated with a negative result. A tumour mutation alone does not imply inherited predisposition.

Even when the target is present, treatment selection remains a clinical decision. In adjuvant decisions, variant-specific evidence and updated recommendations are especially important: the breadth of a regulatory authorisation and the preference expressed in a guideline do not necessarily coincide.

Sources for this section: ESMO — Cutaneous melanoma: Clinical Practice Guideline (2024/2025); NCBI ClinVar — BRAF c.1799T>A, p.Val600Glu; EMA — Tafinlar: current product information and patient leaflet.

The early trials: BREAK-3 and METRIC

In 2012, BREAK-3 compared dabrafenib alone with dacarbazine in 250 patients with advanced BRAF V600E melanoma. The initial publication reported median progression-free survival of 5.1 versus 2.7 months. The finding contributed to drug development, but describes monotherapy and a historical chemotherapy comparison.

That same year, METRIC assessed trametinib alone against chemotherapy in 322 patients with advanced BRAF V600E or V600K melanoma. Median progression-free survival was 4.8 versus 1.5 months. These studies cannot be compared as though they had assigned the same patients to the two molecules.

Retaining these milestones helps explain how treatment evolved. Their contribution differs from the earliest trials: they directly concern dual BRAF/MEK inhibition and its role in advanced disease.

The analysis published in 2019 on five-year outcomes among combination-treated patients documents that some can achieve prolonged control. It does not mean every response lasts years, or that a complete radiological response automatically equals cure. Baseline disease characteristics and the patient’s condition affect interpretation.

These trials also have defined comparators and inclusion criteria. COMBI-AD studied dabrafenib and trametinib against placebo in 870 patients with completely resected stage III melanoma and a BRAF V600E or V600K mutation.

The final 2024 analysis confirmed benefit for recurrence outcomes; overall survival in the full population did not reach the specified statistical significance. This distinction must remain explicit: reducing recurrences and demonstrating an overall-survival advantage are not the same outcome.

Selection requires balancing baseline risk, expected benefit, toxicity and alternatives. Variant-specific analyses need cautious interpretation alongside guidelines; identical benefit cannot be promised to every subgroup. Follow-up remains necessary after treatment has ended.

Sources for this section: Long et al. — COMBI-AD, final results (2024); ESMO — Cutaneous melanoma: Clinical Practice Guideline (2024/2025).

Brain metastases: interpreting COMBI-MB correctly

COMBI-MB is a phase II study with several cohorts, not a randomised comparison against every other treatment. In the main cohort of 76 patients with BRAF V600E, asymptomatic brain metastases and no previous local brain therapy, an intracranial response was observed in 58% of participants. Duration of control remained an important limitation.

This figure must not automatically be extended to symptomatic patients, those receiving corticosteroids, people with a different variant or other clinical circumstances. Older percentages for dabrafenib alone and particular subgroups do not describe the combination in all patients.

Management requires integration with neuro-oncology, surgical and radiation assessments where relevant. In the study population, survival favoured the sequence starting with immunotherapy.

This is clinically important information, but not a rule to apply without considering the individual case. Symptoms, growth rate, need for prompt control, brain metastases, overall health and ability to receive immunotherapy can alter the balance. Not every trial participant reaches the planned second treatment.

A useful question for the centre is therefore: why is this sequence appropriate for me, and what criteria will trigger reassessment?
