It should not be described as conventional cytotoxic chemotherapy. BRAF and MEK belong to the same cellular signalling pathway: blocking it at two points can improve tumour control compared with BRAF inhibition alone.

This biological rationale is not enough to select treatment. Clinical trials measuring benefits and risks and a relevant tumour mutation are required. The authorised melanoma combination is cobimetinib with vemurafenib; its components cannot be freely replaced with other medicines in the same classes. Prescribing remains the responsibility of an experienced oncologist.

Sources for this section: EMA — Cotellic: indications, product information and leaflet; Larkin et al. — coBRIM, randomized phase 3 (2014).

Current indication and limitations

In the European Union, Cotellic is indicated with vemurafenib in adults with unresectable or metastatic melanoma positive for a BRAF V600 mutation. Testing must precede selection: the lesion’s appearance, age or skin location cannot establish mutation status. Assessment includes stage, symptoms and previous treatments.

This indication is not an adjuvant authorisation after any excision and must not be transferred to ocular melanoma or other diseases without specific evidence. Immunotherapy alternatives may also exist in BRAF-mutated melanoma. The decision should therefore explain why this pair is proposed, what it aims to achieve and how it fits into the overall treatment pathway.

Sources for this section: EMA — Cotellic: indications, product information and leaflet; ESMO — Cutaneous melanoma: Clinical Practice Guideline (2024/2025).

What coBRIM demonstrated

The randomised phase III coBRIM trial enrolled 495 patients with advanced BRAF V600 melanoma, comparing vemurafenib plus cobimetinib with vemurafenib plus placebo. The 2014 publication documented improved progression-free survival, meaning time without worsening or death, with the combination.

The initial report gave medians of 9.9 and 6.2 months; these figures belong to that specific analysis. Updates with different follow-up can report different medians, as in EMA documentation: these are not necessarily contradictory findings. Tumour response, response duration, progression and overall survival must be distinguished.

The comparison does not prove superiority over every current treatment: the control was vemurafenib alone, not a modern immunotherapy regimen. A trial median is not an individual prediction. Benefit must also be assessed alongside toxicity, which does not disappear when the second medicine is added.

Sources for this section: Larkin et al. — coBRIM, randomized phase 3 (2014); EMA — Cotellic: indications, product information and leaflet.

Adverse effects and planned monitoring

Effects to understand include diarrhoea, nausea, rashes, fever, photosensitivity and abnormal liver tests. Muscle-enzyme elevations and, less commonly, serious muscle problems can occur. Marked muscle pain, unusual weakness or dark urine should be reported without waiting for the next appointment.

Cardiac-function monitoring and attention to eye symptoms are also relevant: blurred vision, spots in the visual field or other changes require prompt assessment. Treatment can cause serious events and require interruption or modification decided by a doctor. Do not automatically attribute every symptom to the medicine: infection and other causes must be considered.

The schedule of tests and visits is individualised. Keeping the centre’s contact details, an updated medication list and written symptom-reporting instructions helps make care safer. In emergencies, seek emergency services and communicate the treatments currently being taken.

Sources for this section: EMA — Cotellic: indications, product information and leaflet; EMA — Zelboraf: indications, product information and leaflet.

Administration, interactions and daily life

Cobimetinib and vemurafenib have different administration instructions: follow the calendar prescribed by the centre.
